In consideration of the acceptance of the registration as a student in the course, I hereby for myself, my heirs, executors, administrators, and assigns, waive, release and discharge any and all rights I may have against naturotherapist/trainer: David Lee, his partners, employees, sponsors, officers, assistants, teachers and employers and further release and save harmless naturotherapist/trainer David Lee, as well as all Vital Health employees, sub-contractors from any damages which I may suffer as a result of taking the course and from also carrying out the bio assessment work on clients.
In the future when you are using the device in your health/fitness practice, if the Quantum Resonance Analyzer device or program malfunctions, we are not liable for repairs, replacement of parts or device. Vital Health is not liable if there has been incorrect consultation or assessment given to client and hence the clients has suffered injury in any way in their health and physically functioning due to the consultation from the Quantum Resonance Analyzer.
I further acknowledge that I have to follow all instructions applicable to conducting my activities in a safe and prudent manner.
