The client agrees to receive the specified number of consultations and training sessions with David Lee or other Vital Health Team members in exchange for the stated fee.
The content, duration, and frequency of sessions are governed by the Terms and Conditions outlined in this agreement.
No Refund Policy
• Refunds are not available for sessions within the Health Assessments Package or Therapy/Nutrition Package, including the report.
Cancellation Policy
• A 24-hour notice is required to cancel a session.
• Failure to provide notice will result in the client being charged the full session fee unless the situation is beyond the client’s control.
Freezing Session Times
• Clients may freeze their sessions for up to one month.
• Freezing beyond one month will void the contract, and the client forfeits the remaining sessions unless due to serious illness, surgery, or at the discretion of David Lee or the subcontractors.
Refund Policy for Fitness and Nutrition Packages
• Refunds are available within 10 days of signing the agreement, minus completed sessions and a $180 administrative fee.
• No refunds are available after 10 days.
Waiver of Liability
• The client waives, releases, and discharges David Lee, Vital Health employees, and affiliates from any liability for damages incurred during the coaching, fitness, therapy, or nutrition programs.
• The client confirms they are physically and mentally fit to participate, with approval from their physician.
Client Responsibility
• The client agrees to follow all instructions to ensure activities are conducted in a safe and prudent manner.
Agreeings of this agreement constitutes your express consent and permission for Vital Health to use your name, photographs and responses for promotional and advertising purposes to promote Vital Health and its programs, in any media and in any manner whatsoever, without limitation or restriction, and such consent is deemed given by your signature of Agreement. All information and materials submitted become sole and exclusive property of Vital Health and may be used in promotions and advertising.
